Lower Blepharoplasty with Fat Grafting: Male Patient Case Study

In this video, we’ll discuss a case of a male patient who had significant under-eye bags, making him look constantly tired and older than his actual age. He underwent transconjunctival lower blepharoplasty combined with fat grafting to achieve a natural, youthful appearance without an overdone or hollowed-out look.

Patient Background & Concerns
The patient, a middle-aged man, had large under-eye bags due to excess fat accumulation and mild skin laxity. He was particularly concerned about looking older and fatigued despite feeling energetic. He wanted a subtle yet effective solution that maintained his masculine features.

Procedure Details
We performed a transconjunctival lower blepharoplasty, meaning the incision was made inside the lower eyelid, leaving no visible scar. This approach allowed us to remove and reposition the fat while preserving the natural eyelid structure.

To prevent a hollow or sunken appearance, fat grafting was performed. A small amount of purified fat from another part of his body was injected into the tear trough and upper cheek area, creating a smooth transition between the lower eyelid and cheek.

Results & Recovery
The patient had some swelling and mild bruising, which subsided within 10–14 days. His final results became apparent after a few months as the fat settled and integrated naturally. His under-eye area appeared rejuvenated, with no unnatural hollowness or stiffness.

This case highlights how a customized approach using transconjunctival blepharoplasty and fat grafting can achieve a natural, refreshed look, especially for male patients.
