Crawl through a gap in the Nurses lounge to enter the women's locker room, then head up the staircase to the third floor where you’ll find a medical dummy in exam room 4. Use the nearby instrument of force to remove the arm, then break down a wall and go down the ladders in room D1.
Go through the medical records room then pull the Key of Bliss out of the wall. Head back to the medical dummy and use it to get the Lapis Eye Key. Next, Grab the Directors key from room L3 then use it to open the Directors office. Climb the ladder and activate the generator.
Head to room 17 and power up the stretcher to reveal the number 9659. Go back to the chained box and enter that on the keypad. Next head to the restroom and climb through the gap into room L1. Break through the M3 room wall to find a clue.
The first number is 1 and is given at the top of the document. The second is when a loud hour comes which is when you turn the clock hand to 2. Be careful here though because it does get a bit spicy. The third is the number of pills in the sink which is 6, then the final number is how many times you see a light flicker through a gap which is 2. That gives you a code of 1622.
Head back to the chained box and enter those numbers which will open the box.
